SSA Forms for Your SSDI Appeal
The right forms at the right time. Here's every form you need for each stage of the appeals process.
Tip: You can file most appeals online at ssa.gov/apply/appeal-decision-we-made. If filing by mail, download forms from ssa.gov/forms and send to your local Social Security field office.
Stage 1: Reconsideration
Form SSA-561 — Request for Reconsideration
The primary form to start your appeal. Tells the SSA you disagree with their initial decision and want a new reviewer to look at your case.
Form SSA-3441 — Disability Report — Appeal
Provides updated information about your condition since your initial application. Include new doctors, treatments, and changes in your condition.
Form SSA-827 — Authorization to Disclose Information
Allows the SSA to request your medical records from doctors and hospitals. You'll need this at every appeal stage.
Stage 2: ALJ Hearing
Form HA-501 — Request for Hearing by Administrative Law Judge
Filed after a reconsideration denial. Requests a hearing before an ALJ at your local Office of Hearings Operations (OHO).
Form HA-4632 — Claimant's Medications
Lists all current medications, dosages, and prescribing doctors. Submit this before your hearing date.
Form SSA-1696 — Appointment of Representative
If you hire an attorney or representative, this form officially appoints them to act on your behalf with the SSA.
Stage 3: Appeals Council
Form HA-520 — Request for Review of Hearing Decision/Order
Filed after an unfavorable ALJ decision. Asks the Appeals Council in Falls Church, VA to review the judge's decision for legal errors.
Stage 4: Federal Court
There is no specific SSA form for federal court appeals. You file a complaint in the U.S. District Court within 60 days of the Appeals Council's decision. This requires an attorney — federal court procedure is complex.
General Forms (All Stages)
Form SSA-3368 — Adult Disability Report
The original application form. If you're starting a new application rather than appealing, this is the main form you'll fill out.
Form SSA-795 — Statement of Claimant or Other Person
A general-purpose form for providing written statements to the SSA. Useful for explaining your situation in your own words.
⚠️ Deadlines Matter
You have 60 days from the date you receive each denial notice to file the next appeal. The SSA assumes you receive mail within 5 business days of the date on the notice. If you miss the deadline, you generally have to start over with a new application.
